How the Care Navigation System Works

A clear framework designed to help caregivers move from uncertainty → to understanding → to confident decision-making with structured support.

Step 1: Build Awareness and Understanding

Step 2: Navigate Decisions in Real Time (Core Support System)

Step 3: Access Enhanced Support for Complex or High-Acuity Situations

Most caregivers naturally operate between Steps 1 and 2 as their ongoing support system.

Choose the level of support that fits where you are right now.

This is where clarity begins. You start by learning how to recognize what matters medically, what to pay attention to, and how to make sense of what you’re seeing and hearing in real time.

You begin to:

  • Recognize early warning signs and meaningful changes
  • Distinguish between what is expected vs what needs attention
  • Understand common patterns in aging and medical complexity
  • Feel more grounded when interpreting medical information

This step reduces the sense of confusion and constant uncertainty that often comes with caregiving.
This is the center of the caregiving experience — where most people are actively making decisions, communicating with providers, and responding to changing medical situations.

Here, the focus shifts from understanding information to knowing how to use it.

You begin to:

  • Think clearly through medical decisions as situations unfold
  • Know what questions to ask and what information matters most
  • Communicate more effectively with healthcare teams and specialists
  • Apply structured frameworks to reduce overwhelm and second-guessing
  • Translate complex medical information into clear next steps

This is the core support system most caregivers rely on throughout their caregiving journey. It provides a consistent way to approach uncertainty, so you are not making decisions in isolation or reacting without a clear process.
Some situations require additional support — especially when conditions are changing quickly, decisions are time-sensitive, or care becomes more medically complex.

In these moments, you may need more direct guidance and closer support.

This level provides:

  • Increased access to personalized support when needed
  • Deeper review of evolving medical situations and care decisions
  • Guidance through hospitalizations, discharge planning, and care transitions
  • Help navigating high-stakes or rapidly changing scenarios

This step is designed to provide added structure and responsiveness when situations require a higher level of attention.
These two levels provide the foundation for understanding, decision-making, and day-to-day caregiving navigation.
Most caregivers aren’t missing information — they’re missing a way to interpret it

Tier 3: Intensive Care Navigation + Concierge Support

This is your ongoing support system for navigating the healthcare world with structure, clarity, and confidence. Designed for caregivers who are actively making decisions for a loved one, this membership helps you move from uncertainty and overwhelm into a grounded, step-by-step way of thinking through what to do next.

Includes:

  • Everything in Tier 1
  • Weekly coaching sessions focused on real-time problem solving and decision support
  • Case-based guidance so you can apply principles directly to your current situation
  • Priority access to Q&A support for timely questions between sessions
  • Practical frameworks for communicating with medical teams and specialists
  • Tools for recognizing risk, escalating concerns, and making informed decisions under pressure

This membership is where knowledge becomes usable. Instead of trying to interpret complex medical situations alone, you’ll have a consistent, structured space to bring what’s happening and work through it with expert guidance and peer support.

Most members find this level to be the turning point — because it replaces second-guessing with a clear process for thinking, asking, and acting in real time.

Tier 2: Medical Navigator for
Caregivers Membership

This membership is designed for caregivers who want to better understand the medical changes that often occur as parents age. Through educational resources and monthly group discussions, you’ll learn how to recognize early warning signs, understand common medical patterns, and feel more prepared when health concerns arise.

Members have access to a monthly group Q&A session where caregivers can ask general questions and learn from shared experiences. You’ll also receive access to a growing library of recorded trainings and educational materials focused on helping caregivers recognize problems earlier and navigate medical situations with greater confidence.

This tier is ideal for caregivers who want ongoing education and awareness, even if they are not currently facing an urgent medical situation.

Tier 1: Caregiver Foundations Membership

This tier is designed for caregivers navigating serious illness, hospitalization, rapid changes in condition, or highly complex care systems where consistent, personalized support is essential.

Includes:

  • Everything in Tier 1
  • Bi-weekly strategy session tailored to your specific situation
  • On-demand check-ins within defined response windows
  • Personalized review of clinical scenarios and care decisions
  • Direct support through hospitalizations, discharge planning, and transitions of care
  • Advocacy preparation for critical conversations with medical teams

This is the most comprehensive level of support. It provides a steady, experienced lens during high-stakes moments so you are not left to interpret complex or rapidly evolving situations alone.

The focus here is stability, clarity, and guidance when the margin for uncertainty is small — helping you stay grounded, informed, and effective when it matters most.

When Caregivers Typically Reach Out

Most caregivers seek support during moments when the healthcare system suddenly becomes more complicated or decisions feel harder to navigate alone.

Common situations include:

• A parent or loved one has had a recent hospitalization or emergency room visit
• You’re receiving mixed or confusing information from different providers
• You’re unsure whether a new symptom or change in condition is serious
• Discharge instructions or follow-up plans feel unclear
• You’re worried about missing something important
• You’re trying to be proactive but don’t know what to watch for or prioritize
• You feel like you’re constantly reacting rather than making clear decisions

Many caregivers wait until situations become overwhelming before seeking support.

This work is designed to help you recognize patterns earlier, think through decisions more clearly, and navigate the healthcare system with greater confidence and structure.

About

Built from real-world clinical experience inside the healthcare system

This work was created from direct experience inside the healthcare system — where decisions are made quickly, information is often fragmented, and caregivers are frequently left trying to interpret complex situations on their own.

In my clinical work with older adults in hospital settings, I saw a consistent pattern: families were being asked to make important decisions without a clear framework for understanding what was happening or what to do next. Even highly capable caregivers were often left feeling uncertain, reactive, and overwhelmed.

This system was built to change that.

The Care Navigation framework is designed to help caregivers move through medical complexity with more structure, clarity, and confidence — not by giving more information, but by helping you understand how to apply what you’re already being told in real time.

The focus is practical:
What matters right now. What to ask next. What to watch. And how to think through decisions when things feel unclear.

You don’t need to become a medical expert to navigate this well. You just need a clearer way to interpret what’s happening and a structured way to respond.

That is what this work is here to support.
